Trainee Health Care Assistant (Health Services) @ The Aga Khan University Hospital

You will be responsible to:

• Assess and document patients' vital signs
• Report oral intake and any other abnormalities to the supervisor
• Provide hygiene care
• Ensure to have consent and appropriate documentation in the consent form before initiating the services
• Coordinate with safety and security department as required
• Assess home environment for patient and staff personal safety
• Provide supportive care / comfort care to patients at home (if applicable)
• Coordinate patient care and concerns with Home Health multidisciplinary team as required
• Support a smooth transition of patients from hospital care into home care.
