Job Summary

The Aga Khan Health Service, Pakistan is seeking a seasoned and Dynamic Nurse for its Karachi Region under Elderly Care Project.

Role and Responsibilities:

  • Conduct household sample collection and provide home health services to elderly patients, as identified by project staff
  • Nurses will be responsible for adhering to all protocols related to sample collection, including proper handling, and ensuring safe preservation and transportation of samples in accordance with standardized procedures.
  • Adhere to all protocols related to sample collection, including aseptic techniques, proper labeling, timely documentation, and correct procedures for venipuncture or other biological sample collection.
  • Educate participants and their caregivers on post-sampling care, potential side effects, and when to seek further medical attention.
  • Undertake training as provided by project staff
  • Nursing assessment, diagnosis, planning, intervention and evaluation of patients
  • Perform comprehensive patient and environmental assessments related to home health needs
  • Administer medication via different routes (oral, IV etc.) as needed
  • Keep organized logs of household visits and schedule of visits
  • Demonstrate efficiency in the use of electronic monitoring equipment
  • To ensure updated medical records of patients visited
  • Able to follow and fill healthcare algorithms and maintain log sheets
  • Develop care plans in consultation with clinician and identify referral requirements
  • Plan daily home visits of patients, coordinate and implement care delivery while anticipating patient care needs across the continuum, utilizing nursing process (assessment, diagnosis, planning, implementation and evaluation)
  • Provide individual and family patient education, counselling
  • Is aware about palliative care and provide support to the patients to limit hospitalization
  • Recognize prescriptions to understand medication dosage, complications, contraindications, and interactions
  • Perform head-to-toe assessments of elderly population and direct Nursing assistants for supportive care
  • Connect patient with consultations to report any complications or issues to the physician
  • Be familiar with signs and symptoms of infection and be comfortable in dressing wounds and assessing long-term catheters
  • Collaborate closely with the physician, nursing assistants, and patient to ensure medications, lifestyle changes, and therapy services are suitable. 
  • Identify if the patient is facing any psychological or mental health issues and connect with specialized care as needed.

 

Required Skills

Qualification & Experience:

  • Nursing Diploma/BScN/post-RN BScN & Valid PNC license
  • 1-2 years of work experience in clinical settings, particularly in sample collection, Training or experience in preventive and palliative care, as well as geriatric nursing care would be preferable.
  • Certification in the following areas is preferable: Basic Life Support (BLS)/Safe Medication Administration/Intravenous (IV) Cannulation/Nasogastric (NG) Tube Insertion/Foley’s Catheterization.
  • Ability to assess, plan and manage patient care with expertise as per protocols
  • Excellent interpersonal skills, including communication skills
  • Able to use digital applications on tablets, mobile, laptops
  • Basic Computer Skills/Microsoft Office
