Assist Case Managers in coordinating and monitoring client cases and support services.
Maintain accurate and up-to-date case files, records, and documentation.
Schedule client appointments, meetings, assessments, and follow-up visits.
Prepare, organize, and file reports, correspondence, and case-related documents.
Collect and update client information in case management databases or information systems.
Communicate with clients, service providers, and partner organizations to facilitate service delivery.
Track referrals, appointments, and follow-up actions to ensure continuity of care and support.
Support the preparation of case summaries, reports, and statistical data.
Ensure all documentation complies with organizational policies, donor requirements, and confidentiality standards.
Required Skills
Skills and Qualification:
Bachelor's degree in Social Work, Psychology, Sociology, Public Health, Business Administration, or a related field.
Previous experience in case management, social services, healthcare, humanitarian organizations, or administrative support is preferred.
Proficiency in Microsoft Office (Word, Excel, Outlook) and case management information systems is an advantage.
Excellent written and verbal communication skills in English.
